Both Rootsweb and Ancestry.com have the US SSDI Social Security Death Index. If you find a record of interest in the index, a letter is automatically generated that you can print on your printer and send it to the Social Security Administration to obtain a photocopy of the actual application form SS-5.  An example of the letter follows;
